A new Frailty Pathway was introduced in January 2017 with the opening of a new 38 bed facility at Orpington Hospital. Churchill and Elizabeth Wards care for patients that need a little more support with their treatment before they are ready to return to their usual place of residence. Similar to the Proactive Pathway, this support includes input from medical, nursing and therapy staff plus social and voluntary services.
The unit will provide short term hospital care for frail patients living in the community who need to be assessed and helped to get back to more independent living, or who have been in the Princess Royal University Hospital and who need some additional support to prepare them to leave hospital.
At the centre of the new pathway is the Consultant Gerontology Hotline available to GPs. GPs can use the hotline to get specialist advice and guidance, arrange a stay in the Orpington unit or a rapid same day comprehensive assessment undertaken at either Orpington or the Princess Royal University Hospital
