Bromley Well, a new service to help support residents to remain independent, gets underway in October. Bromley Well brings together a variety of individual support services in a partnership of voluntary organisations from across Bromley’s Third Sector.
Existing services and contracts are being brought together and, at the same time, an additional £1.5m is being invested, with the Council and Bromley NHS Clinical Commissioning Group (CCG) jointly funding these services.
All of these caring services focus on the needs of different service user groups in Bromley and, through support and self-management, enable individuals to remain independent in the community for as long as possible. The services include support services for: carers; older frail residents; those with long term conditions; people with learning disabilities; people with physical disabilities; and mental health support services. The Council and the CCG are bringing together all of the existing services and contracts and increasing these with additional £1.5m funding.
Councillor Diane Smith, Executive Councillor for Care Services and Public Health said, “We have a brilliant carers community in Bromley who provide invaluable care for their loved ones. These services will enable them to access the support they need to continue this vital role. The increased funding is good news and as well as providing continuity of provision, a ‘one stop shop’ approach for a range of other support services is being introduced. The outcomes will mean that more residents will be able to live more independently in their local communities, with the risk of any emergency admissions to hospital being reduced.”
Councillor Smith added, “There clearly will be a transition from the existing providers and Bromley Well will be working to make sure that this is as smooth as possible, with service users being contacted to advise them personally of the changes.”
Dr Andrew Parson, GP and Clinical Chair of NHS Bromley CCG said, “Bromley Well is a new and innovative approach to help local people stay well and remain as independent as possible for longer. We are committed to working in a more joined up way to support people, especially those who are more vulnerable or maybe at risk of a crisis. By having a single point of access, people will get the right help to meet all of their needs from the various services on offer through Bromley Well. This is another example of how we are transforming the way joined up care is provided for our residents.
The new arrangements and contract starts on the 1 October 2017 and are being provided through a partnership of local service providers called the Bromley Third Sector Enterprise, made up of Age UK Bromley and Greenwich, Bromley and Lewisham Mind, Bromley Citizens Advice Bureau, Bromley Mencap and Community Links Bromley. The Council and the CCG have worked closely with the new partnership to ensure that carers services continue to be provided at a similar level to what is currently available as well as taking account of what carers needs are likely to be in the future.
