The CCG is committed to working with patients, the public, Healthwatch and the voluntary sector to make sure that they are able to influence the services that we plan.
We have a Patient Advisory Group (PAG), made up of local people. The PAG is always our first port of call when we are asking for people to get involved in our work.
In the summer of 2015, we held a patient workshop with our PAG, Healthwatch and Community Links Bromley to test our thinking on our Out of Hospital Strategy. Their feedback was used to inform this strategy and the development of the Integrated Care Networks (ICNs).
In January 2016, we held a focus group with some of our PAG members who either have long term conditions or care for people with long term conditions. This helped to influence the design of care pathways within the ICN.
In May 2016, we held a patient workshop to test our plans further and get views on the pathways and how we will test how successful the ICNs are. Following that workshop and further discussions with clinicians, we refined our proactive care pathway which will operate within the ICN model. It focuses on providing integrated care to patients identified as having high needs and complex long term conditions.
In November 2016, we held a focus group with members of our PAG and Healthwatch Bromley to share our plans on the frailty pathway and the opening of an integrated unit at the Orpington Hospital for people who are frail. This session enabled us to respond to a range of questions about the new service and also helped us with the production of clear information for patients who will use the service.
In the summer of 2016, we also ran a public survey to inform the development of the ICN model and received 129 responses. Outcomes of the survey were used to further influence our plans.
