An integrated care network (ICN) is a model of care that brings together a range of health and care services to work in a more joined up way to provide care for patients. This model means that services can be more responsive to the needs of patients and make sure they have all the care they need to keep them well.
We have three integrated care networks in Bromley, each covering around a third of the population. Our ICN map below shows which GP practices are in each one.
Services are being brought together within each of the ICNs so that care will be more co-ordinated.
The model was developed in partnership with clinicians, health and social staff and patients.
In each of the three integrated care networks is a multi-disciplinary team (MDT) providing care for patients. The MDT is made up of GPs, hospital doctors, nurses, therapists, and care navigators. Social care is arranged if required. These teams are working together to care for and support patients with complex health conditions and their families.
