Integrated care

Integrated or ‘joined up care’ describes a process which makes it easier for staff from different organisations to provide services that work more closely and effectively together.

In Bromley we believe that integrated care makes a huge difference to our patients, especially those who have complex health conditions and long term conditions. It transforms the way they receive care and how this is arranged for them.

We are in a great position to deliver integrated care in Bromley. All our local health providers including GPs, the hospital, community services, mental health, end of life care and the voluntary sector had signed a formal agreement to deliver joined up care together in Bromley through our Integrated Care Network model.   

The NHS Five Year Forward View sets out a clear direction for the NHS to develop new models of care that will provide more integrated services. To make this happen, barriers between hospital, community and primary care will need to be removed so the focus is on patients and systems of care rather than individual organisations.  We are committed to this in Bromley and making sure we provide the best possible care for our population
