Important information about Charterhouse Surgery

Date: 21 August 2019

The primary care services that have been provided by McLaren Perry Ltd. at Charterhouse Surgery will come to an end on Monday 30th September 2019. Patients will no longer be able to access primary care medical services from the practice after this date.

All patients registered at the practice have been contacted by the CCG to explain the reasons for the closure, and how to register with an alternative GP Practice. 

Patients registered at Charterhouse Surgery will now need to register with another GP practice that covers their home address before Monday 30 September 2019.
