SURVEYS: Freshstart (exercise referral) and Heartsmart (cardiac rehabilitation) Programmes: how was your experience?

The Freshstart (exercise referral) programme and Heartsmart (cardiac rehabilitation) Programmes, provided by Mytime Active, are approaching the end of the contract period.

As a current, recent or past participant of one or both of the programmes, we would be very grateful if you can share your experiences to help us shape these services.

 Your feedback will help to ensure that any future services meet the needs of people attending in Bromley.

  • Please email BROCCG.PatientQuery@nhs.net or call 0203 930 0100 if you would like a printed version of either survey, along with a FREEPOST envelope to return completed surveys
  • You can also complete either survey over the phone by calling 0203 930 0100
  • Please let us know if you would to complete the survey in any other format and we will do our best to help you
