Our healthier SE London

Our Healthier South East London is a five year commissioning strategy which aims to improve health and integrated care across south east London.

The programme is led by the six NHS Clinical Commissioning Groups in south east London – Bexley, Bromley, Greenwich, Lambeth, Lewisham and Southwark – with commissioners from NHS England (London), working in close partnership with local authorities; local people and patients; local providers of care and other partners.

Our Healthier South East London focuses on priority health issues which need collective action to address successfully. Its goals are to improve health, reduce health inequalities and to ensure the provision of health services that meet safety and quality standards consistently and are sustainable in the longer term.

This is likely to mean that the way in which some health services are delivered will change, with more care provided in community settings outside hospital and a greater focus on helping people to stay well, making services more joined up and making sure that everyone gets the care and outcomes they expect from their NHS.

Why change things?

There are a number of challenges, many of them common to other areas and some specific to south east London:

  • Too many people live with preventable ill health or die too early
  • The outcomes from care vary significantly and high quality care is not available all the time
  • We don't always treat people early enough to have the best results
  • People's experience of care is variable and can be much better
  • Patients tell us that their care is not joined up between different services
  • The money to pay for the NHS is limited and need is continually increasing
  • Every one of us pays for the NHS and we have a responsibility to spend this money well

In south east London, we have some very good health services. People are living longer and many people are healthier. But we also have some services that could be better. We have services that people find hard to access, and some people don't get the help they need to keep themselves and their families well.

You can read about these challenges in more detail in the full case for change South East London Commissioning Strategy Programme. This document is labelled a draft in progress as it is subject to review. An updated version will be published by summer 2015.
