In September 2015, we published our Out of Hospital Strategy. The aims of the strategy are to improve health, prevent ill health, increase the independence of patients and improve their experience of care. This strategy was informed and developed in partnership with clinicians, patients and other health and social care partners. It sets out our commitment to and plans for delivering integrated care provided outside of hospital. This included the development of integrated care networks and the first focus on vulnerable patients with complex health needs.
Since publication, we have made great progress. We have worked closely with people who care for patients and with patients directly who have been part of designing the models of care.
