We have a number of programmes in place to empower patients with specific health conditions to manage their conditions and self-care.
We commission patient education programmes for people with diabetes (both types 1 and 2). If you have diabetes, please speak to your diabetes nurse or GP who can advise you on how to access these programmes.
In 2016, we set up our Heart Support Group. This was established in response to feedback from patients and supports those with a heart condition by providing talks and advice from clinical specialists from across Bromley. The CCG has helped the group form a patient led committee and get accreditation from the British Heart Foundation.

We are working with the Bromley Breathe Easy Group in collaboration with the British Lung Foundation to support people with respiratory conditions. We provide key speakers and information about local services.
As part of our CCG wide on improving cancer services and outcomes, we are planning a patient group to support people with cancer and enable them to have a voice in local services.
